I did this trek in 1983. I think the Pokhara to Manang side was only opened to trekkers in 1979, so i saw it in the early days. I was 20 then and I'm 62 now. It was one of the highlights of my life. I can't remember where i started the trek but i don't think the road went up as far as Besisaha. Also there was no road on the Muktinath side back then. You had to walk down the Kali Gandaki gorge and all the way back to Pokhara

Start at Chame, anything before that is a dusty walk along the worlds largest building site.
